There is a solid Contributor Agreement in the 
Game Dev Kit at www.gamedevkit.com (not free but cheap!)

But the "back end rights" part sounds like a 
custom job to me and you should probably get a 
lawyer to do it for you.  Otherwise you may not end up with what you want.
