I'm sure there are some general articles on this somewhere on the web, but if you look at the Game Definition File Editor utility in the DirectX SDK you'll see the currently supported ratings systems. The default rating system is chosen based on the Windows locale information, but the user can select whatever rating system they want. The game developer provides ratings for all the systems they are rated for, and show up as "unrated" for all other systems.
The reality is that a large number of titles world-wide of all kinds will be "unrated". Parents using Parental Controls who decide to block "unrated games" will likely need to use the per-title approval functionality for many games, particularly depending on their chosen rating system.
