[games_access] Survey

Hi Adam, yes that helps. Well, as a SIG we're mainly focused 
on getting developers to create accessible games for the 
disabled more than the demographics of the dev community. 
However, it would be nice to know how many in the dev 
community has, say, visual, hearing, mobility, and cognitive 
disabilities and how the industry has (or has not) created a 
friendly work environment (ie, what accommodations are 
provided to them so that they can do their job).
