[games_access] Game Accessibility Survey updated

Good work so far. Few points I can still see:

1. Fixed font size on IE5 means I can't enlarge the font size with my 
browser. Definite accessibility issue in itself.
2. No ALT text on graphics (i.e. PHP Surveyor). Not a major issue as that 
seems to be the only graphic.
3. The entire survey is not particularly accessible for learning disabled 
gamers. The writing is too difficult, with no images or symbols to get 
across questions.
